I am choosing to participate in Death & Soup, a gathering led by Jennica Mills that may include ceremony, conversation about death, grief and loss, somatic and nervous system practices, gentle movement, time outdoors on uneven ground, and a shared meal.

I understand that this gathering is not therapy, medical care, or a substitute for either, and that Jennica Mills is not acting as my therapist or medical provider. I am responsible for my own physical and emotional wellbeing during the gathering, and I will pace myself, rest, or step away at any time.

I understand that the themes we explore may bring up strong feelings, and that participation in any practice, conversation, or ceremony is always voluntary. If I have a physical, psychological, or medical condition that may be affected by participation, I have considered it and, where appropriate, consulted my own care providers.

Food will be served. A vegetarian butternut squash and white bean soup with homemade sourdough bread. The soup is gluten-free but contains dairy; the bread contains gluten. I understand the meal is prepared in a private home kitchen and cannot be guaranteed free of allergens or cross-contact, and it is my responsibility to disclose allergies and make my own decisions about what I eat.

I agree to hold in confidence what others share during the gathering, and I understand that no photographs or recordings are made during ceremony.

To the fullest extent permitted by law, I release and hold harmless Jennica Mills, along with any assistants and the owners of the property where the gathering is held, from claims, liabilities, and expenses arising from my participation, except in cases of gross negligence or willful misconduct.
